Fairfield Glade Clinic to Discontinue Primary Care Services

COOKEVILLE-Cookeville Regional Medical Center announces that primary care services will no longer be offered through the Fairfield Glade Clinic in the Fairfield Towne Center after Friday, November 15, 2013.

The clinic has offered primary care services as well as provided office space for specialty physicians.

“We did not make the decision to discontinue primary care services there lightly,” said Paul Korth, CEO at Cookeville Regional. “It has been our pleasure to serve the residents of the area and we hope that they will continue to choose Cookeville Regional for specialty care services. As a matter of fact, we are currently working on other arrangements to ensure that our specialty physicians will continue to have a presence in the Crossville area.”

Specialty physicians from Tennessee Heart and Tier One Orthopedics will continue to see patients at the Fairfield Glade Clinic location.

Patients who have visited the clinic for primary care services may obtain their medical records by visiting the clinic through Friday, November 22, 2013 or calling 931-484-3050. After that date, their primary care records will be available at the Cookeville Regional Medical Group office located at 128 North Whitney in Cookeville.
